Deposits are used to reserve the appointment time and will be applied towards rates, fees, or copays. Deposits are refundable if canceled more than 24 hours in advance. Deposits will be applied towards the late cancellation fee if missed, canceled, or rescheduled with less than 24 hours' notice. Appointments will be considered missed if not present within 15 minutes from the scheduled time.
Unused credit will be applied to future rates, fees, and/or copays or refunded.
